Opatz and Struzyk purchased their first care center in May of 2012 from Paynesville Area Health Care Center. Today they own and operate many independent living, assisted living, memory care and skilled care facilities. Prior to the start of Premier Health Care Management and later on CURA, Opatz worked for a large not for profit senior care organizations in a variety of roles including Vice President of operations and Director of Real Estate development. During his 10 year he developed and constructed nearly 200 million in development in senior living. He has extensive experience with financing projects to include bonds, bank placement and HUD. He is a Licensed Health Services Executive with a bachelor’s degree in public administration with an emphasis in health care administration from St Cloud State University.
Fred Struzyk is a Licensed Health Services Executive. Fred served as an administrator of health care facilities for the past 39 years. Hospitals, Nursing Homes, Clinics and Assisted Living facilities located in Central Minnesota. These facilities have ranged from independent to nationally chained based organizations. His passion has been Long Term Care. Areas of expertise is nursing home and assisted living reimbursement and daily operations. Fred is Co-Owner of CURA and Premier Healthcare Management. Fred earned his degree in Business Management at Concordia University, St. Paul.
David is back with Premier Healthcare LLC after a personal business adventure. He has run communities in Minnesota and Colorado and got his start in the profession at Hilltop Healthcare Center and Garden View at Hilltop in Watkins. Before becoming a nursing home administrator, he was a Firefighter Paramedic and Flight Paramedic and has been in healthcare for over 12 years. David earned his degree from the University of Minnesota in Healthcare Administration.
Michael joined CURA and Premier Healthcare LLC in January 2025, bringing over six years of experience in healthcare administration, with expertise in both assisted living and skilled nursing facilities. Prior to his healthcare career, Michael worked in international sales, living in China and Toronto, Canada, before returning to his home state of Minnesota. He earned his degree in Healthcare Administration from the University of Minnesota.
Kardell has a Bachelor of Science in Nursing from Minnesota State University, Mankato. Kardell is passionate about serving the healthcare needs of our seniors and has 19 years of experience in long term care, serving in both direct care and leadership positions. The last 10 years Kardell has served as Director of Nursing of a large Central MN long term care facility.
Ludwig joined Premier Health Care Management in 2016 and served as the Director of Nursing for 5 years before transitioning into Regional Clinical Director role in 2021. Ludwig has 24 years experience working in senior services in various roles. Ludwig has an associate's degree in Nursing from Lake Superior College. Megan has achieved four deficiency free surveys for skilled care centers from the Minnesota Department of Health.
Alexander Huls holds a Bachelor of Science degree in Exercise Science from Southwest Minnesota State University and an Associate Degree in Nursing from Minnesota State Community and Technical College. He is passionate about advancing quality care in both acute and long-term care settings, with over 10 years of experience as a Registered Nurse. Alexander has served in direct care and leadership roles, including time as a state surveyor with the Minnesota Department of Health.
Beth Swanson began her healthcare career over 12 years ago, working as a nursing assistant in hospice care while pursuing her nursing degree. She earned her Associate Degree in Nursing from Hibbing Community College and her Bachelor of Science in Nursing from The College of St. Scholastica. Beth has spent her entire RN career in skilled nursing, serving in a variety of roles including floor nurse, nurse manager, Assistant Director of Nursing, MDS Coordinator, and Director of Nursing. She has over three years of experience as a DON, where she led clinical teams and supported regulatory success. Beth is passionate about problem-solving, creative thinking, and helping care teams reclaim meaningful time with residents.
